42% of Americans Are Vitamin D Deficient — And Most Have No Idea

The most widespread nutritional deficiency in the United States is not iron. It is not calcium. It is vitamin D — and according to the most comprehensive national survey data available, it quietly affects nearly half of all American adults. The extraordinary thing is not that researchers know this. The extraordinary thing is that most of the people who have it have never been told.

What the National Data Actually Shows

A landmark analysis published in Nutrition Research, led by researchers Forrest and Stuhldreher, examined data from the National Health and Nutrition Examination Survey (NHANES) and found that 41.6% of American adults had 25-hydroxyvitamin D levels below 20 ng/mL — the threshold most clinicians use to define deficiency. The numbers were even more striking in specific populations: 82.1% of non-Hispanic Black adults and 69.2% of Hispanic adults tested deficient.

A more recent analysis published in the journal Cureus by Parva and colleagues, also drawing on NHANES data, confirmed that deficiency remains endemic across the American population regardless of geography. The authors found that risk was elevated in older adults, in those with limited sun exposure, in those with higher body mass, and in individuals with darker skin pigmentation — which naturally requires more UVB exposure to produce the same amount of vitamin D.

In 2007, Dr. Michael Holick of Boston University — widely considered the world's leading vitamin D researcher — published a review in the New England Journal of Medicine estimating that over one billion people worldwide were deficient or insufficient. That number has not improved in the intervening years.

Why This Goes Undetected for Years

Vitamin D deficiency is a quiet condition. Unlike iron-deficiency anemia, which produces fatigue dramatic enough to prompt a clinical visit, or hypothyroidism, which affects weight and energy in recognizable ways, vitamin D insufficiency tends to produce diffuse, non-specific changes that are easy to attribute to aging, stress, or poor sleep.

Common subjective reports among individuals with low vitamin D levels include persistent fatigue despite adequate sleep, a general sense of low energy during winter months, and changes in mood and motivation that many people simply accept as part of modern life. Because none of these experiences point unambiguously to a single cause, most people never connect them to a missing nutrient — and most clinicians do not routinely screen for vitamin D status unless a patient presents with a bone-related complaint.

Dr. Holick has repeatedly noted that the global vitamin D deficiency pandemic represents one of the most underrecognized health problems of our era. In the absence of routine testing, most people live for years — sometimes decades — without knowing their status.

Understanding What Vitamin D Actually Does in the Body

Vitamin D is not technically a vitamin in the classical sense. It is a prohormone — a precursor that the body converts into an active hormonal compound. The vitamin D receptor (VDR) has been identified in virtually every tissue type in the human body, from the immune cells of the bone marrow to the neurons of the brain. This distribution suggests that vitamin D plays a role far beyond its historically recognized function in calcium metabolism.

The active form of vitamin D, 1,25-dihydroxyvitamin D (calcitriol), regulates the expression of hundreds of genes. Research from the vitamin D laboratory at UC San Diego, led by Dr. Cedric Garland, has identified over 900 genes that vitamin D influences, many of them involved in the regulation of cell growth, immune function, and tissue repair. The scope of this influence helps explain why deficiency has been associated in observational research with such a wide range of health outcomes — though it is important to note that association is not causation, and individuals should consult qualified health professionals before drawing clinical conclusions from these findings.

What is established beyond reasonable dispute is that vitamin D is an essential biological input for which no adequate dietary substitute exists. The body was designed to obtain it primarily through UVB exposure to the skin — a process that modern indoor living has severely disrupted.

The Testing Gap: Who Is Actually Checking?

The 25-hydroxyvitamin D blood test is inexpensive, widely available, and capable of identifying deficiency with clinical precision. And yet, routine vitamin D screening remains uncommon in most primary care settings in the United States. A 2019 analysis in the Journal of the American Board of Family Medicine found that vitamin D testing was ordered for only a small fraction of patients during routine wellness visits, and that ordering rates varied dramatically by physician, practice, and region.
